We’re pleased to announce that the Together 2 Goal® Diabetes Symposium, in collaboration with the American Diabetes Association, will be held September 12-13 in Indianapolis. The in-person meeting will focus on how medical groups and health systems can advance the management of people with Type 2 diabetes by implementing evidence-based care processes. Peer-to-peer learning and networking opportunities will be highlights of the symposium.
The Together 2 Goal® Diabetes Symposium will occur in conjunction with the Chronic Care Roundtable (September 11-12), AMGA Analytics’ Analytics for Improvement (A4i) Learning Collaborative meeting (September 12-13), and AMGA’s Chief Medical Officer/Medical Director, Chief Nursing Officer, and Quality Leadership Council meetings (September 13-15). Please note these additional meetings are open to AMGA members and invited guests only.
The Together 2 Goal® Diabetes Symposium is open to all AMGA members, including Together 2 Goal® Primary and Quality contacts, as well as members of the Chief Medical Officer/Medical Director, Chief Nursing Officer, and Quality Leadership Councils, and the A4i Learning Collaborative participants.
Other attendees may include ACO Directors; Care Coordinators; Care Managers; Chief Medical Officers; Chief Nursing Officers; Chief Operations Officers; Chief Quality Officers; Clinical Quality Coordinators; Diabetes Physician Champions; Diabetes Program Coordinators; Endocrinologists; Managers and Directors of Care Innovation, Population Health, Quality Improvement, and Clinical Quality Programs; Medical Directors; and Quality Committee Chairs.
