National Day of Action
Spacer35
August 2018 Edition

This year, we are bringing National Day of Action to you! On November 8, we’re encouraging you to assemble your diabetes team for a T2G Talk & Taste. During this time, we ask that you watch a two-minute T2G video and discuss its application with your team over breakfast or lunch. Your meal is on us (up to $100)! For this day, we’ll provide you with a downloadable T2G Talk & Taste kit that will include:

  • Annotated PowerPoint presentation
  • T2G provider video
  • Discussion guide
  • Certificates for staff recognition
  • Additional resources
  • Reimbursement form

Kits will be available on our website mid-September.

If you can’t hold your T2G Talk & Taste on November 8, then aim for any other day that week and it will still count! Let us know if your team plans to participate—email your name, organization name, and T2G Talk & Taste date to together2goal@amga.org. To receive reimbursement for your meal, send us a picture of your event and completed reimbursement form by November 30.

Questions? Email us at together2goal@amga.org.

Campaign Spotlight

Last month, representatives from 10 Together 2 Goal® participating organizations met in Nashville to launch the Eye Care Cohort of the Innovator Track. Participants had a lively discussion about tactics to improve frequency and documentation of retinal scans for patients with diabetes. They also developed action plans, which they will implement in their home organizations over the course of the yearlong initiative.

Resource of the Month

Did you know that diabetes is the leading cause of blindness for working age adults? Many people living with diabetes don’t understand how their condition can affect their vision. The National Institutes of Health (NIH) has developed 5 Things You Should Know About Diabetic Eye Disease, a one-page handout to explain this risk. Print this out and share it with your patients today.
