Ten T2G campaign participants gathered in San Diego, CA last month to kick off their involvement in the T2G Diabetes Bundle Best Practices Learning Collaborative. This intensive learning collaborative is designed to help participants advance the quality of care they deliver by improving their performance on the bundle measure. Participating groups have access to peer organizations, an advisory committee of clinical experts, and AMGA staff to expand upon their existing campaign efforts and accelerate the pace of their improvement. The collaborative participants include:

Campaign Spotlight

Last month, T2G was featured in an American Association of Diabetes Educators (AADE) blog post, titled "Sharing Success Stories: 5 Strategies for Increasing DSMES Referrals." Written by one of our campaign participants, Sutter Health's Valerie Spier, M.P.H., RDE, CDE, the post highlights best practices gleaned from implementing the "Refer to DSME & Support Programs" plank at her organization. Now in its third year, their referral program is making an impact, resulting in an overall increase in referrals to their SHIDEN Diabetes Education Program by more than 6% last year. Resource of the Month

Did you know that the American Diabetes Association (ADA) Standards of Care is a living document? Notable updates are incorporated throughout the year in response to important events like the approval of new treatments or the publication of new findings. Informed by the recent ADA consensus statement, the April update incorporated a recommendation for individualized assessment to determine appropriate nutrition plans for patients. The most recent updates focusing on improving cardiovascular and renal health in people with Type 2 diabetes were based on findings from the Canagliflozin and Renal Outcomes in Type 2 Diabetes and Nephropathy (CREDENCE) trial.
