Campaign Data Results
All medical groups and health systems participating in Together 2 Goal® commit to reporting diabetes prevalence and control rates in their patient populations on a quarterly basis as part of joining the campaign.
Through reporting and analysis, the campaign will be able to:
Evaluate progress toward the campaign goal of improved care for 1 million people with Type 2 diabetes by 2019
Provide blinded feedback and comparative reports to campaign participants
With a rich history in the facilitation of data-driven collaboration to improve population health, AMGA Analytics is working with Together 2 Goal® for data reporting and measurement.
The campaign’s measurement specifications and value sets have been updated! The 2.0 version of the measurement specifications can be viewed here. We have also provided a summary of changes which can be viewed here as a quick reference guide to the updates. The 2017 HEDIS Value Sets for Together 2 Goal® Measures 2.0 referenced in the updated measurement specifications can be downloaded in Excel format here. Participating groups have two options for reporting data on a quarterly basis:
To submit your data, you can use the campaign’s data collection portal to enter the data directly into a reporting interface.
Or, data for the measures can be entered into an Excel template and attached to an email sent to DataForT2G@amga.org. To request or download the Excel template, or for a copy of the data reporting User Guide please contact us.
